2009 - 2010  INDEPENDENT CONTRACTORS*

 All individuals are independent contractors and must sign a w-9. They will recieve a 1099 at the end of the year. They must also be trained in CPR and the Heimlich Maneuver and submit a recent, police and FBI finger print report.  If a report was obtained recently, it must not expire before the current school year .  City Administrator level is based on experience.

City Administrator-level 2* will earn ____per Student** that is tutored at any site(s) in their school district or surrounding school districs or a percent of it based on amount of hours the student completes.

City Administrator-level 1* will earn _____per Student ** that is tutored at any site(s) in their school district or surrounding school districts or a percent of it based on amount of hours the student completes.

Site Coordinator* will earn ___per Student they recruit to be tutored at their site or a percent of based on the amount of hours completed by the Student. If they get someone else to recruit Students for the site, they are responsible for negotiating an amount, out of their earned money, that they pay to that individual.

Site Location Providers* (Church, profit or nonprofit)  will receive compensation as a General Administration Fee.  This is for each student** that completes the full 9 week paid sesion tutored at the Site Provider's location or a percent of it based on the student'd completed hours.

Tutors* earn _____per hour per student, 1.) They can tutor up to five students** per hour 2.) They can tutor either math or reading.   3.) They must submit a recent state and FBI finger print report and it must not contain any violations contained on form FB 701).    4.) They must submit a copy of their degree or if no degree, a copy of their transcript credits (minimum 60 semester hours or 72 quarter hours).

Students who have attended all 36 hour/9 weeks tutoring sessions and have improved 25% between the pre-test and the post-test, will recieve a bonus gift certificate.

Snack* a  fee is paid for each student per day up to 18 days.

Transportation*  a fee is paid for each student per day up to 18 days.

Transportation Aide* a fee is paid per student per day up to 18 Days

*Snack, Transportation/Transportation aide are seperate contracts
